Community Approaches to health care during and after crisis situation

This thesis deals with issues of community care with regard to community health care during and after emergencies. The theoretical part aims to introduce the general topic of community care. The first chapters of this thesis are dedicated to community care and utilization of community nursing care, including the history and development of community care in the Czech Republic as well as outside its territory. The thesis further provides a summary of institutions, health and social services within community care, including the possibility of home care. It also deals with the role of community nurses and their involvement in the process of community planning. Another important part of this thesis is the issue of emergencies where the term "emergency" is defined, and divisions of emergencies and the cooperation of integrated response system are described. The thesis furthermore concentrates on the procedures of the ambulance service during emergencies including emergencies with polytrauma and the subsequent treatment in health-care facilities. One of the goals of this thesis is to describe the role of medics during emergencies and the role they play in caring for patients coming from different communities. Further chapters deal with specific community health care: elderly people, children, people with disabilities, groups of different cultures - ethnic and national minorities. It also deals with the specifics of providing health care from the point of view of religion. The practical part is dedicated to quantitative survey of knowledge of medics in issues of community health care across different levels of education and different ranks. A research question was determined: What is the understanding and professional level of medics in the community care setting during and after an emergency? The respondent data was obtained in the form of a questionnaire and results are shown in diagrams. The questionnaires aimed to find out whether the respondents had ever heard the term "community care" and to find out their theoretical knowledge on this issue. However, the main point was to evaluate how vast is their practical experience.
THE ROLE OF CHILDREN´S NURSE IN SCHOOL CATERING

Approaches to nourishment and eating habits are formed from early childhood under the influence of family lifestyle, gradually together with the influence of school and society where a child grows. Healthy lifestyle should systematically address all children during the whole school attendance, adequately to their age and needs. Comprehensive link between theoretical and practical influencing of pupils in subjects Human and Health and Education to Health with the possibility of link to school canteen. Then we will be on the good way to cooperation of all the involved parents, healthcare staff, teachers and school canteens leading to the common goal, which is GOOD HEALTH. The aim of the thesis was to assess the role of a children nurse in school environment, namely in school catering. The research tried to analyse catering standards for pupils of primary schools and to find the experience and satisfaction with school catering among parents and to map approaches of children of lower and higher level of primary school to school canteen meals. Quantitative and qualitative research was applied to the thesis. Data collection was performed by means of questionnaires for parents and children from the 3rd to 9th grades and focus group among children of the 1st and 2nd grades of primary school in Jindřichův Hradec. The questionnaires were processed and statistically evaluated by SPSS programme. We applied Parkinson's chi-squared test, a paired sample t-test for graph and table outputs. The focus group was performed in classrooms and in the canteen during break and during lunch at presence of a teacher and the vice headmistress. The research sample consisted of parents of children attending primary school in Jindřichův Hradec and pupils of the 1st to 9th classes of the same school. School catering, which is a frequent topic among professionals as well as the wide public in the whole Czech Republic and the results of our research are obviously an exception. Analysis of catering standards has shown that parents can only partially participate in preparation of menus, as they partially participate in financing of school lunches. From observation of 1st and 2nd class children we have found that children of this age are affected by the social environment they are in during the day. We have found that nearly all the children are satisfied with their meals. Pupils of 3rd to 9th classes formed another group. They commented and assessed canteen meals, where our sample differed from the other researches within the whole Republic. Satisfaction with canteen meals did not substantially differ with age as we expected. The research into satisfaction with school meals among parents has shown that parents of elementary school children are more satisfied that those of the higher level children, so the satisfaction among parents differs between the lower and higher primary levels. The research points out the connection of educational and healthcare systems, that there would be a place for a children nurse in this community, who operated in the school healthcare service before privatization of primary healthcare and still successfully operates there in many countries. It is necessary for community nurses to implement, lead and coordinate the care and for their care to be perceived as legitimate and essential in all spheres. The research results were requested by the vice headmistress of the school in Jindřichův Hradec in advance for possible correction of school catering and they might be also presented at seminars or conferences focused on nutrition and healthy lifestyle of children.
Options in further training of home care agency nurses

This diploma thesis deals with the possibilities of further education in the agencies of domiciliary care. This is a very current topic. Like any other profession, the nursing profession requires a particular education. The development of the new methods in the medical treatment and home care demands more thorough and specialised education of the nurses employed by the domiciliary-care agencies. The aim of the thesis was to find out the present situation of the nurses´ education in the domiciliary-care agencies, the professional specialisation of nurses, the possibilities of education, the nurses´ need of further education and the employers´ willingness to enable the nurses of the agencies to take part in the further education in their field. It can be said that the aims of the research have been fulfilled.
View therapist of necessity for an ad'vancement community health care in Czech Republic

Community nursing services provide health care services in communities outside health care institutions. They are targeted at the sick people in their own home environments, especially providing disease prevention, health support and health care assistance. The goal of community health services is to improve the whole community health, especially of those people who are exposed to risky lifestyles and who are under risk of petting sick, or of dech, and to provide a specific care to them. The introductory part of my thesis contains the theory and elementary parts and types of the health care system in the CR as well as categorized structures and functions of the World Health Organization. It also includes the primary health care goals as well as the goals of the European Union and of the Czech Ministry of Health for the health care system development. In the second part the theory deals with the meaning of the community care, its foundations, the past and the present in the CR. It also describes the nurses´ job descriptions. In the course of the quantitative research I chose the technique of a questionnaire on the grounds of a stratified selection. It was made up by questions used in interviews with nurses. In the frame of qualitative research I chose a method used in interviews with workers in health insurance companies in which I used a guided interview. The goal of my thesis was to find out whether health care workers are aware of health care deficits. I asked for the workers´ opinions on the necessity of community care development in the CR. The goals of my work were reached. The research question was used to find out what the of health care insurance companies representatives´ opinions on the necessity of the community care in the CR are. On the grounds of the questionnaire it was found out that the representatives of health care insurance companies have a positive opinion on the necessity of the community care development in the CR. I did not prove the first hypothesis which stipulated that nurses working outside hospitál facilities do not feel any deficit in providing health care services referring to the provided care. The second hypothesis was proven stipulating that nurses working in hospital facilities can see the most serious deficit in the the fact that people lack their interest in their own health. The third hypothesis was proven, too. The health care workers´ opinions on the the necessity of the community care in the CR are positive.
Possible utilize of nurse's competenicies in community health care in Czech Republic

The philosophy of community care is deeply rooted in an opinion that states that one of the main missions in a cultivated society is helping the people who, due to their disease, are not able to to meet their own demands and their needs vitally necessary for life. Community care means providing an intervention and support at the right level and in the right form for clients, so that they can reach the highest possible level of self-sufficiency and control over their own lives. Recently, there have been many changes in the health care system. The goal of community care is to provide thourough care in clients' home environments and thus enable them to stay with their community as long as possible. The major task is providing phychical, psychological, paliatic and spiritual services. Families are the vital part of the care and a fundamental root of the whole team cooperation within the system of community care. The community teams consist of health care and social workers, community volunteers, the sick or the disabled. Nurses are important members of multidisciplinary teams. They take an active part in community assessment and they also help ensure a complex care for citizens. The research survey combined a quantitave and a qualitative survey. Firstly, a set of questionnaires for nurses and non-professional public were worked out. Next, an interview was prepared for nurses. The research sample group was made of 153 non-professional respondents and 121 nurses working in different medical facilities all over the Czech Republic. Nine nurses were interviewed. In my thesis two goals were set up. The first goal was to find out where in the community care nurses could implement their competences within the legislative frames. The second goal was to find out what opinion on community nurses non-professional public have. I worke out four hypotheses on the grounds of my goals. The first hypothesis is worded as: The non-professional public is interested in having nurses working for the community care. The second hypothesis is worded as: Nurses between 36-45 of age are willing to work as community care nurses. The third hypothesis is worded as: Nurses' interest in working for the communty care depends on their finished education. The fourth hypothesis is worded as: Nurses working 21-30 years in the health care system are interested in being active as community nurses in home care agencies. On the grounds of the qualitative research survey and five research questions it is possible to set up two hypotheses. The first hypothesis is worded as: Nurses do not know who a community nurse is. The second hypothesis is worded as: Nurses are not interested in working as community nurses. Community care has had a long history. The foundations of community care have been laid in Great Britain. In the Czech Republic community care has not been a novelty. However, we can claim that so far, no community care system has been created.
The School Nurse, and its elocation to the health care system in the Czech Republic

There is a new trend of shifting the provision of health care from hospitals to the patient?s natural environment. We are increasingly aware of the expensive and sometimes ineffective late hospital treatment; therefore, there are stronger efforts for early diagnosis, prevention and health education from an early age. It is necessary to keep the future generations focused on their health and the health of their family members who live in the community. The initiative must originate not only in families but also in the school environment in which children find themselves almost every day. A school nurse is an expert in the field of community care working in school environment. The work load of the school nurse rests in maintaining and improving physical and mental health. The school nurse supports pupils in their responsibility for their health and she creates a system of formalized school activities, prepares and implements the school?s educational program focused on prevention. The data in the research part of the thesis was obtained through quantitative and qualitative research survey. The quantitative research was carried out using the technique of an anonymous questionnaire. The questionnaire was intended for general practitioners for children and adolescents, as well as nurses working with general practitioners for children and adolescents. The qualitative research data was collected through semi-structured interviews with the head teachers of elementary and secondary schools on the basis of which casuistries were created resulting in categorized charts. The objective of this thesis was to find out about the position of general practitioners for children and adolescents, and paediatric nurses working with general practitioners for children and adolescents on introducing the function of school nurses in the healthcare system of the Czech Republic. The research in this thesis showed a low level of awareness about the position of school nurses. The primary controversies related to the introduction of school nurses can be described as follows: the method of financing, the issues of authority of such nurses, and the way of preparing for this occupation. The main positive aspect of school nurses can be seen by the respondents in comprehensive care for pupils, prevention and better public education. School head teachers, general practitioners and nurses believe that school nurses should be placed in schools according to the number of school pupils and school head teachers assume greater use school nurses in primary schools than in secondary ones. The method of financing school nurses should be, according to the respondents, from multiple sources. This thesis may be used as a basis for further research work focused on the position of school nurses, and also as a proposal of establishing the school nurse position in the healthcare system of the Czech Republic, or as informative material for professional as well as lay public.
Contribution of children nurse{\crq}s work to community nursing both within the Czech Republic and abroad

My dissertation is aimed at the following questions: System of children care, organization of health-care policy in the Czech Republic, competencies and abilities to children care, document Health for Everybody in 21st Century, community nursing, nurse interventions in particular age categories, and my own research proceedings. The research section of the work contains both quantitative and qualitative research procedures. The data were collected of a questionnaire and semi-structured interview. The examined set of a qualitative research consisted of 9 respondents {--} 5 nurses and 4 medical practitioners for children and youth. The quantitative data were collected with an interview method. The questionnaires were designed for the unprofessional public {--} parents of children who are up to 18. The examined set of the questionnaire research consisted of 73 respondents (100%). The research proceeded from mid June 2008 to the end of July 2008 and its results have been processed into category tables and graphs in the program of Microsoft Office Excel 2007. I especially try to find the opinion of children nurses and children medical practitioners on introduction of a community children nurse in the Czech Republic and specify the functions that a community children nurse can perform in the Czech Republic when working with children community. I also try to find locations at which the community children nurse could work, and obstacles hampering to establish this new working position within the Czech Republic. Last but not least, I am interested in the public interest in the nurse{\crq}s affect to health protection and care for children. So far, there has not been a community nurse position established as an independent community professional in health protection and care for children within the Czech Republic. However, in other countries, community nurses work in the state interest and they form a part of the public health-care system. They are rewarded for their work, which is highly appreciated, from the public budget. Development of the community health-care is a WHO priority, and it has already established its firm position in the Czech health-care conception. As the main advantage of introduction of a community nurse I consider the fact, that both children and parents would have a better chance to use a wider variety of health-care services at school, in their household or at the doctor{\crq}s consulting room. Community nurse{\crq}s activities could help to inform families and to improve the health-care edification level. They could also assist the families that need highly intensive health and social care. Because of independent work of community nurses, nurse{\crq}s profession would become more attractive and nurse{\crq}s social position would improve.
The necessity of visiting duty of primary care nurses at children´s patients

Visiting service is a lawful activity of nurses working in paediatric departments. During the visiting service there is a possibility to obtain valuable information on the child{\crq}s family and its background. A nurse recommends the parents how to prevent diseases or injuries at their children. A nurse also educates them about a suitable diet, hygiene and upbringing. During the visiting service there is enough time for parents to ask about their children{\crq}s prevention and upbringing. A community nurse can stand in for this post of primary care nurse as well. Such kind of nurses has not been initiated yet in the Czech Republic. The Ministry of Health is going to include this kind of nurses in general educational programme.
